Community Health Center of the North Country's Substance Use Care Coordination program, also known as our Road To Recovery program, is a free and confidential service available to ALL members of our communities, whether you are a patient at one of our health centers or not. Our team will see those at risk of becoming addicted, already addicted, in recovery, and family members of those fighting addictions. Adults and children of all ages are welcome to utilize our services, which are offered throughout both St. Lawrence and Franklin counties.

The Road to Recovery does not need to be taken alone, and CHCNC is here to help.
